If your plan has not been formally terminated, you will need to print out this amendment and sign it. The trustee of the plan, as well as an authorized officer of the company sponsoring your plan, must sign where indicated. Once it is signed, please place it inside your plan binder. Your plan will be out of compliance if this amendment is not signed (adopted) and kept with the main plan documents. NAFEP cannot provide legal or tax advice so if you need help in understanding the contents of this amendment or how it may affect your plan, please seek an independent advisor to discuss it with you.
